FAMILY SERVICE COORDINATOR (FSC)
JOB DESCRIPTION
Division:Families F.I.R.S.T. Date: September 15, 2009 Salaried Non Exempt
Performs multiple tasks in assuring requests from CFS Specialist and/or Court Order are channeled to the appropriate resource, while orchestrating all resources towards effective quality service delivery.
Responsible for orchestration of resources for quality service delivery.
Adhere to Person/Family Centered practice at all times.
Maintain fidelity with the chosen EBP/PP models.
Effective communication and working relationship with CFS Specialists, supervisors, managers and team regarding child/youth and family.
Accuracy of information taken and given.
Fosters teamwork while coordinating delivery of services.
Monitors and ensures safety throughout the life of the case.
Responsible for initiating Family Engagement with service team.
Ensures accuracy of assessments completed on children/youth and families.
ESSENTIAL FUNCTIONS OF THE POSITION AND PERFORMANCE STANDARDS
Accepts, review and expand on initial assessment for family.
Uses discretion in conducting needs assessment while encouraging and utilizing family voice/choice and family driven principles.
Participates in and supports safety plan.
Responsible for coordination and providing services designed to meet the individualized needs of children/youth and families.
Prepares draft copy of court reports and testifies as needed, providing expert testimony regarding case plan progress, child/youth and family strengths and barriers as well as related decisions.
Coordinates MH/SA related documentation and incorporates into family driven plan and overall service delivery.
Responsible for completing all required documentation is completed and provided to DHHS-CFS within the designed time frames.
Responsible for organizing and facilitating Family Team Meetings on a monthly basis (or more frequently as needed) focusing on Safety, Permanency and Well Being.
Supervises overall delivery and coordination of services through implementation in the event of in-home or out-of-home placement and adjusts as needed to ensure progress.
Coordinates with the family in developing their permanency goals by encouraging families to select individualized strategies and interventions that meet their needs.
Coordinates with children/youth and families in identifying, securing and working with informal support systems when appropriate to assist in meeting their individualized needs and case plan goals.
Maintains on-going contact with Families F.I.R.S.T Manger, CFS, and Family Partners regarding each case assigned as deemed appropriate or as requested.
Authority to select/provide services that will address critical domains of the YLS/CM for youth involved with the Juvenile Justice system.
Authority to selects services in conjunction with children/youth and families support skill development of the youth/family
Drafts updates of the YLS/CM.
Visits with children/youth and family a minimum of monthly in accordance with Federal requirements.
Authority to provide information to CFSS if negative reaction occurs regarding critical incidents experienced within or by children/youth or families.
Works collaboratively with other team members and uses own discretion to control, monitor and manage safety plans with families.
Responsible for conducting protective capacity assessments and other assessments to assist in the elimination of a safety threats.
Responsible to oversee and provide assurance of routine and emergency medical care for child/ren.
Coordinates Independent Living (Adolescent Permanency) and Transitional Living services to children through age 21.
Conducts ICJ home studies, ICPC to assure supervision and support to children.
Maintains documentation information on N*Focus.
Takes applications for guardianship/ adoption, along with relinquishment request and completes subsidy request where appropriate.
Ensures collaboration with CFSS to ensure permanency hearings are held within 12 months post removal of the children along with subsequent reasonable effort hearing.
Coordinates aftercare services for a period of 12 months post discharge and reports children/youth and family status to DHHS-CFS as requested within required timelines.
Responsible for making recommendations for case closure.
Performs other duties as may be assigned.

Supervises: N/A
Internal: Works in harmony with all team members, supervisors, managers and other related personnel.
External: Continual contact with families, CFSS, Service Coordinators, and other stakeholders involved with the child/youth and family.
JOB SPECIFICATIONS AND WORK REQUIREMENTS
Education
Required: Minimum of Bachelor Degree in Human Services or related field.
Desired: Masters Degree in Human Services, Counseling or related field.
Experience
Required: Must be 21 years of age and have a minimum of 3 years experience as a Family Partner or Team Leader, or three years experience working with children and families.
Desired: Prefer prior case management experience.
Licenses, Certifications, and/or Registrations: Clear Criminal History Background and Adult/Child Abuse checks, as well as all program specific certifications, NCI, First Aid/CPR, Car Restraint, Defensive Driving, clearance from all background checks and drug screens (Trainings offered by Visinet, Inc.)
Equipment/Tools/Work Aids: Must have fluid knowledge of computers to include N*Focus, Microsoft Word, Excel, etc.; professional phone skills to receive and relay pertinent client or case information

Specialized Knowledge and Skills: Prior case management and/or field experience in DHHS helpful. Spanish, sign language, etc. beneficial.
Working Conditions:
Varied climates both indoors and outside in homes, neutral locations or related areas.
Some travel to attend meetings and court held appointments.
