Position Description

Purpose

 

Identify, secure, and process all current and new Family Mentor resources, providing assistance with training, consultation, initial licensing, renewal licensing, updates and monitoring files to ensure all are in accordance with the requirements set by the Nebraska Department of Health and Human Services (DHHS), Federal guidelines, Visinet and all related contracts.  

 

Essential Functions of the Position and Performance Standards
Duties/Responsibilities:
1.       Utilize an outlook calendar daily to illustrate appointments.
2.       Return emails within twenty-four (24) hours. 
3.       Return office messages and cellular messages within one half hour.
4.       Collect all necessary pre-licensure documentation and assignments, placing them in the Family Mentor file in appropriate time frames as outlined in the Program Specific Manual.
5.       Meet weekly with Resource Manager to discuss the following:
a.       Report on the number of new families screened and scheduled for training;
b.       Report on the number of new families with first time placements;
c.        Report on specific resource development goals assigned each week;
d.       Report on the number of licensed homes unavailable as a resource and the identified measures taken to bring the foster home into compliance with Visinet Inc. expectations;
e.        Report on current recruitment activities and the effectiveness of each strategy. 
6.       Will meet with various Family Mentors, Family Mentor committees, associations, and other stakeholders as directed by the Resource Manager.
7.       Provide the potential Family Mentors with information and exercise relevant to the special behaviors and needs of children placed in foster care.
8.       Adhere to Visinet, Inc.’s Family Mentor Training model in the primary teachings to the potential Family Mentors.
9.       In the event a potential Family Mentor is already licensed, obtain appropriate releases from their previous agency or entity and complete an updated Visinet, Inc. authored home study.
10.    Review license expiration dates and assist Family Mentors in securing the necessary training to renew their license within 60-90 days of license expiration; maintain files indicating dates of contacts and dates of training with potential Family Mentors. Notify Family Mentors three (3) months prior to their expiration dates. 
11.    Assist with providing home studies to the Resource Manager for review two (2) weeks before the license expires.
12.    Assist with the completion of new home studies or other licensing job duties as necessary. Assist with transfers and amended home studies.
13.    Communicate to the potential Family Mentors the need to identify any history of criminal involvement, arrest, convictions, involvement with Child or Adult Protective Services and/or involvement with the Department of Motor Vehicles.
14.    Participate in PRIDE training as directed by the Resource Manager.
15.    Maintain and update an aftercare support schedule for foster families in between placements
16.    Complete internal assessments of Family Mentors as assigned by the Resource Manager.
17.    Keep Family Mentor phone list updated with expiration dates.
18.    Collaborate with other Resource and Training team members to schedule and plan various activities for the Family Mentors and children.
19.    Ensure that the Health Insurance Portability and Accountability Act (HIPAA) standards are maintained throughout the program.
20.    Consistently review Continuous Quality Improvement (CQI) plan and ensure the requirements are being met on a daily basis by all employees.
21.    Support and promote programs within Visinet, Inc. and mediate conflicts with community contacts to preserve the reputation and perception of consumers and stakeholders.
22.    Maintain a basic working knowledge of the Visinet, Inc. Families F.I.R.S.T. Model to accurately convey the program and expectations to interested parties.
23.    Other duties (not limited to the above) as determined by Visinet, Inc. Administration.
 

 

1.       Education
a.       Required: Bachelor’s degree in social work or related field; excellent writing skills;                                 
b.       Desired: Bachelor’s degree in social work or related field and two years of experience completing home studies.
 
2.       Experience
a.       Required: Previous work with at-risk families;
b.       Desired: Previous recruiting and public speaking/marketing.
 
3.       Licenses, Certifications, and/or Registrations: Completion of all Visinet, Inc. regular and ongoing training identified for the position.
 
4.       Equipment/Tools/Work Aids: Must be knowledgeable in Microsoft Office Programs including Word, Excel, and Outlook or similar types of software (MDaemon, Google Apps, etc.).
 
5.       Specialized Knowledge and Skills:
a.       Ability to demonstrate professionalism in the administration of duties;
b.       Strong interpersonal, communication skills;
c.        Ability to work with minimal supervision.
 
6.       Working Conditions:
a.       Varied, internal as well as external; Family Mentor homes; kinship homes of children/youth in homes which could challenge your value system at times; 
b.       Ability to work under pressure and meet strict deadlines.
